Infection Prevention Control Partners Program (IPCPP):
The IPCPP is an innovative and bold pilot program created to support Post-Acute Care settings across the country. This is a collaboration between a selective group of participating LTC Facilities and is managed by IPCnurse LLC. The goal of the program is to provide boots on the ground level of assistance aimed at closing some of the Infection Prevention Control gaps that exist at the facility level. The collaboration is tailored to adhere to the facility corporate Policies and Procedures and is aligned with all State and Federal Requirements of Participation (RoP’s) to ensure regulatory compliance. The program in part, provides support to facilities that are ready to strengthen their Infection Control initiatives, antibiotic stewardship, and management of MDRO’s with targeted coaching and feedback.
Your center-based Infection Preventionist collaborates with their assigned IPCnurse Consultant to maintain a robust IPC program and ensure best practices. Regular meetings ensure e.g., that elements for a successful antibiotic stewardship program are being met. These meetings also facilitate shared education opportunities such as learning modules and outcome data review and feedback.

prevention practices (F945).Remote monitoring of EHRs for potential infections, ABT
3-day timeouts and appropriate antimicrobial
utilization.Infectious Disease Outbreak Alerts - Your assigned
IPCNurse Consultant works with their laboratory vendor to obtain facility testing data reports for UTI’s, Cdiff and other MDROs. The data is used in outcome surveillance,
determination of Incidents/prevalence and validating
compliance with all reporting requirements.24/7 access to your assigned IPCnurse, in addition to
clinical resources, Medical Directors, and professional
networks.Promotes the use of tools developed by the Centers for
Disease Control and Prevention (CDC), State Department of Health, the Agency for Healthcare Research and Quality (AHRQ), and other partners for
the prevention of HAIs.
